Transportation & Unit Coord. - LTC - Full time -Day w/rotation as needed - Long Term Care
POSITION SUMMARY: The Transportation & Unit Coordinator is a dual role position, serving as the primary link between residents and their medical appointments. Organizes resident appointment schedules, provides safe transport of residents to internal and external appointments, maintains clinical documentation as the Unit Secretary, and provides bedside care in a Certified Nursing Assistant (CNA) role to support the nursing team.

QUALIFICATIONS AND STANDARDS

Education: High school graduate or equivalent required; Pennsylvania Certified Nurse Aide (CNA) registration in good standing required

Experience: Minimum of two (2) years' experience as a CNA required; experience using electronic medical record (EMR) system

Certifications/Licensure: Active Pennsylvania driver’s license with no moving violations, at-fault accidents, major infractions, or license points in the last five (5) years required; CPR required

GENERAL REQUIREMENTS
Physical and Mental Demands/Requirements (subject to modification or accommodation as required):
• Requires ability to lift, carry, push, and/or pull up to fifty (50) pounds frequently; occasionally lift, carry, push, and/or pull over fifty (50) pounds
• Requires full range of body motion, manual finger dexterity, and hand/eye coordination
• Work is primarily sedentary with prolonged sitting on a hard or cushioned chair 70% of the time; standing and/or walking 30% of the time
• Demonstrates near visual acuity with color perception to decipher fine print
• Demonstrates auditory acuity sufficient to hear phone conversations, overhead pages, and alarms
• Must be able to reach above the shoulder, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling
• Demonstrates the ability to work under pressure or stress
• Demonstrates the ability to communicate effectively in both written and verbal form using the English language
• Demonstrates the ability to establish priorities and prioritize tasks to meet deadlines
• Demonstrates the ability and technical skills to work with computers, advanced software including EMR systems, and other office equipment
• Ability to pass facility insurance/MVR requirements; ability to operate wheelchair lifts/securements and other body lifting devices

Working Environment and Hazards:
• Primarily within the facility of FCMC in a well-illuminated, climate-controlled, indoor office environment with average noise level
• This position requires operation of Company vehicles to transport residents to appointments with external/environmental hazards (i.e weather, traffic)
• This position may be exposed to communicable diseases; may be required to wear respirator or other PPE as necessary for protection
• Scheduling hours: hours will primarily be Monday-Friday but flexibility is required as work demands fluctuate
• May work at an offsite location and/or be required to travel to other facilities to attend medical appointments, meetings, and training
• Potential exposure to smoke, hazardous chemicals, and traveling in inclement weather
• Moderate risk of injury from sharp instruments and/or physical/verbal abuse

Reporting Structure: Reports to LTC Director of Nursing. In his/her absence, reports to Nursing Home Administrator.
